To empower you and your family, we provide education, guidance, and resources to make informed decisions about your care. We support and help educate you and your private care team to better understand all diagnoses and treatment plans to create a cohesive path forward. We provide advice and referrals on financial planning, insurance coverage, and legal matters related to aging, such as advance care directives and power of attorney, and offer information on Medicare, Medicaid, and Long Term Care insurance. We have extensive knowledge of community resources and can connect you with appropriate support networks, including home care agencies, assisted living facilities, and support groups. We can also refer you to social services, financial assistance programs, and other relevant resources.
